Interpreting Logs

Below are links to our current interpreting logs. Please make sure you are using the current log when going to assignments. All interpreting logs must be filled out correctly and in their entirety. Interpreting logs are used for invoicing our businesses. Please assure all logs are readable and cropped. You will be asked to redo incorrect interpreting logs.

 
 

CMH Daily Log

  • Please check in and out with Language Services from a CMH phone, call: 53474 (Missouri); 84452 (Kansas); or send a text page to (816) 458-5322 (text page example: Spanish interpreter checking in to CMH Kansas ED.)

  • Log sheets must be completed for each appointment even if the participant is a no show or rescheduled.

  • If your assignment is at the Main Hospital, Broadway Clinics, or West Clinics the log needs to be placed in the pink drop box. If you are unaware of the location of the box, please take your log to the Language Services Office.

    • Main Hospital – The box is behind the right side of the visitors desk. Near the little alcove with the toy wagons for children.

    • Broadway- Behind security desk, thru the door on the right, and down the end of the hall. The box is on the left side of the wall.

    • West Clinics- The box is located to the right of the main entrance.

    • If your assignment is at the South Hospital, the log needs to be placed in the drop box. If you are unaware of the location of the box, you can leave it with Security at the main entrance of the South Hospital.


KUMC Interpreting Log

  • Interpreters must check in with the Interpreter Services Office upon arrival at each assignment and before departure from the premises. Interpreters can call or text from their personal phone or call via office phone for arrival/departure notification. Interpreting Services pager/text line: 913-917-4464

  • Please use the KUMC Extended Interpreting Services log when assignment exceeds scheduled time by 1 hour.

  • Log sheets must be completed for each appointment unless it is cancelled prior to your arrival. The log must have the time in/out for each hands-up interpreting occurrence. The log must be signed by a KUMC employee.

  • For all face-to-face interactions, please make sure the doctor or physician documents the interaction along with your full name before departure.


MVR Interpreting Log

  • Please see the MVR Interpreting Log - Example for additional information.

  • Travel Time - Please list arrival/departure time here in 0.25 (quarter)/15 minute increments.  If you leave your house 1 hour before your job and on your way back it takes you 30 minutes to arrive home or to your next destination, you would put 1.5 hours.

  • Total Hours Billed - This should always have at least 2 hours or more if the job is scheduled for more or runs over.

  • Portal to Portal - Portal to Portal is your Travel Time PLUS Total Hours Billed.

  • Client Signature - Make sure to get the deaf participants signature.

  • Please make sure that all interpreters document the same time on their logs. If times are different, you may be asked to redo your log.
